Surgical Treatment Of infertility

9

a) Varicocele

Varicocelin therapy is a surgeon. The best method of operation is microscopic varicoselectomy surgery. The success rate is higher and the risk of complications is lower than other methods.

b) Obstructive infertility

Transrectal prostate and vesicula seminalis should be performed in patients with obstructive infertility. If there is a cyst that compresses the ejaculate ductus, the obstruction is removed with a simple endoscopic operation. [Type (ED)]

Depending on the level of the obstruction various microsurgical operations are recommended.
a. Vasovaral anastomosis
b. Vasoepididual anastomosis
c. Cross-anastomosis

Non-surgical treatments of infertility

a) Medical treatment

Hyperprolactinemia, hypothyroidism, testosterone excess and deficiency, alpha-hydroxylase enzyme deficiency are compensated by medical treatment.

b) Empirical treatment

At least 25% of infertile men have no cause (idiopathic). A second male patient group does not have any specific treatment, although infertility is diagnosed. Both of these groups are offered empirical treatment.

Drugs used in empirical therapy:
a. Clomiphene citrate
b. Tamoxifen
c. kallikreins
D. Antioxidant treatment
to. Growth hormone therapy

Empirical treatment should be continued for at least 3 months and controlled by spermiograms.

c) Assisted reproductive technologies

Assisted reproductive techniques such as IUI, IVF and ICSI can be used to perform gestation if surgery or medical treatment is not appropriate for male infertility treatment.

IUI: Intrauterine insemination

The sperm from the ejaculate includes washing a contaminating pellet through the cervix and placing it in the uterus.

IVF: In vitro fertilization

Most of the spermatic hurdles in the female reproductive system are removed from the center. Hundreds of thousands of semen are needed in IVF. It involves the accumulation of eggs in the transvaginal way from the ovaries at the time of the ultrasound guidance before normal ovulation. Eggs are fertilized in 500 thousand – 5 million sperm petri dishes. It can be used in moderate to severe infertile men with low number of moving sperm.

ICSI: Sperm micromanipulation

A live sperm is sufficient in ICSI. This technique can be performed with vas deferens, epididymis and sperm taken from the test.

Since IVF and ICSI can be removed from many obstructive processes in natural fertilization process, it is expected that genetic defects causing infertility will be passed on to the baby to be born.
